Both Player and Spectators agree to abide by all tournament regulations and the FNB Junior Golf Tour Code of Conduct posted at www.fnbjuniorgolftour.com.

Player and Parent/Legal Guardian understand membership fees and tournament entry fees are non-refundable.

Both Tournament particpant and his or her parents/legal guardians release the host facilities and Eastern Ohio Junior Golf Tour LLC. DBA The FNB Junior Golf Tour from any liability relating to the player's participation in an FNB Junior Golf Tour event.

Player and parent / legal guardian hereby give Eastern Ohio Junior Golf Tour LLC. DBA The FNB Junior Golf Tour and participating agencies permission to use film, video tape, and/or photgraphs of the above mentioned for lawful promotional purposes.

The Agreement Between Eastern Ohio Junior Golf Tour LLC DBA The First National Bank Junior Tour and Parent or Guardian. To read the over the Liability Release and Waiver Form, please click the link, RELEASE OF LIABILITY.

I understand the $45 membership fee is due by the tournament deadline of the first event my junior golfer competes in.
